The following forms are required for preschool and kindergarten enrollment. Forms must be submitted before your child can start school.

• Registration Form
• Taste of Twos registration form
• Receipt of Parent Handbook
• Parent Agreement Form
• Emergency Consent Form
• DCFS Medical Form – Must be dated within 6 months of the first day of school
• Discharge or Withdrawal
• Religious Education Authorization
• Parent Questionnaire
• Video/Photo Release Form
• Carpool Release Form
• Biting Policy (2′s only)
• The school must have a copy of the child’s birth certificate on file.
